1. In the case of a change of the lawyer’s last name in connection with the conclusion of marriage, dissolution of marriage, change of name, it is necessary to contact the Council and provide the following documents:
– a copy of the certificate of the right to practice law;
– a copy of the lawyer’s certificate;
– a notarized copy of a document confirming the registration of a change of name, surname, etc. (marriage certificate, certificate of name change, court decision on divorce)

2. In the case of the need to obtain a duplicate certificate of the right to practice law in connection with the loss or theft of documents, it is necessary to contact the Council and provide the following documents:
– a copy of the certificate of the right to practice law (if available);
– a copy of the lawyer’s certificate;
– a copy of the lawyer’s passport;
– a receipt for payment for the production of a certificate;
– a receipt for payment for the production of a certificate;
– photo and signature in electronic form on an electronic medium or send to e-mail ( info@radako.com.ua ) after submitting the application and documents to the Council.
– an announcement printed in a newspaper (any official legal publication) regarding the loss of documents indicating P.I.B. the lawyer and the details of the document (“I lost the lawyer’s certificate P.I.B. No. ___ from___, please consider it invalid”);
– in the case of theft of a certificate, a certificate from the police about the registration of the relevant statement (about the application, etc.);
– If the certificate of the right to practice law was issued by the KDKA of the Kyiv region, and not by the Bar Council of the Kyiv region, an extract from the protocol/decision or certificate (KDKA) that issued the certificate to the lawyer with information about the issuance of the corresponding certificate is also added to the list of specified documents!
3. In the event that a replacement is necessary due to the loss of the document’s appearance (rubbed text, wetting, etc.), it is necessary to contact the Council and provide the following documents:
– the original and a copy of the certificate of the right to practice law;
– a copy of the lawyer’s certificate;
– a copy of the lawyer’s passport;
– a receipt for payment for the production of a certificate;
– a receipt for payment for the production of a certificate;
– photo and signature in electronic form on an electronic medium or send to e-mail ( info@radako.com.ua ) after submitting the application and documents to the Council.
4. To produce a certificate of the right to practice law in English, it is necessary to contact the Bar Council of the region to which the certificate was issued and provide the following documents:
– a copy of the certificate of the right to practice law;
– a copy of the lawyer’s certificate;
– a copy of the lawyer’s passport;
– a copy of the first page of the lawyer’s foreign passport;
– a receipt for payment for the production of a certificate;
WARNING! The lawyer applies with a corresponding application to the Bar Council of the region, which issued him the certificate for the first time. In the case of an appeal to the Council in which the lawyer is registered, an extract from the protocol/decision of the regional council (KDKA) that issued the lawyer’s certificate with information on the issuance of the corresponding certificate is added to the list of specified documents!
